Patient Information
Your first visit with us will consist of filling out a new patient form, taking x-rays and a consultation thoroughly explaining your diagnosis and a treatment plan specific to your needs. Please show up 10-15 minutes earlier than your scheduled appointment time to fill out the new patient form and to check any information pertaining to insurance.
You may preregister with our office by printing out our Patient Registration Form. After you have completed the form, you may either email it to aretedental@gmail.com or you may bring it on the day of your visit.
If you have dental insurance, make sure you bring your insurance card with you. If you're not sure if we accept your insurance, just give us a call at 718-460-4099! Please note that we are unable to check for eligibility unless you come to our office in-person due to restrictions of personal information being exchanged through the phone.
If you had recently taken x-rays at another office, you may request them to forward it to our email at aretedental@protonmail.com. However, if additional films are necessary, x-rays will be taken at our facility.
