          - Pinhole Surgical Technique

The Pinhole Surgical Technique is performed to create and augment gingiva, obtain root coverage, eliminate sensitivity and prevent root caries.

The recipient site utilizes no incisions, but just the pinhole from which a full thickness flap is raised that retains the overlying flap of gingival and mucosa. The flap is raised from the existing gingival and mucosa and moved coronally. Bio-resorbable collagen is placed at the site under the overlying flap.

Periodontal diseases are infections of the gums, which gradually destroy

the support of your natural teeth. There are numerous disease entities requiring

different treatment approaches. Dental plaque is the primary cause of gum disease

in genetically susceptible individuals. Daily brushing and flossing will prevent most

periodontal conditions.

Warning Signs of Gum Disease:

  • Red, swollen or tender gums

  • Bleeding while brushing or flossing

  • Gums that pull away from the teeth

  • Loose or separating teeth

  • Pus between the gum and the tooth

  • Persistent bad breath

If gum recession is present and tooth roots are exposed, a gum graft may be recommended.  This procedure is used to control and prevent further gum recession and subsequent damage to the surrounding teeth and bone structure.  Often, a soft tissue graft also improves the appearance of a patient’s smile and sensitivity to heat and cold.

During a soft tissue graft, tissue is surgically placed to cover as much of the exposed root area as possible.  Often, a tissue flap is created to allow the added tissue to be joined to the existing gums to facilitate tissue growth and healing.  This procedure may be used to treat one or several adjacent teeth at the time of the surgery.

Scaling and root planing is a non-surgical procedure, usually performed under local dental anesthesia to improve patient comfort.  The purpose of scaling and root planing is to remove the causative agents which cause inflammation to the gum tissue and surrounding bone – ig. dental plaque and tartar/calculus.  During the scaling process, specialized dental instruments are used to remove dental plaque and calculus from beneath the gums. Planing is the procedure used to smooth the tooth’s root after the scaling process. Root planing helps the gums heal and reattach themselves to a cleaner and smoother root surface.

This patient had gingival recession, inadequate attached gingiva, and abfraction on tooth #12 (in blue circle) and was complaining of sensitivity due to root exposure.
 

Pinhole Surgical Technique was performed to move healthy gum tissue into the treatment area using just a small pinhole, rather than making a large incision and grafting tissue in place.
This technique allows less bleeding, discomfort and post- op pain.
